I just gifted this game to my brother inlaw yesterday. The game loads and runs fine, except for swinging the club.
When you pull the mouse back the club swings back fine, but pushing the mouse forward does nothing, no down swing.
he is running Windows 7

Any help would be appreciated

Tell him to try a controller if he has one. I use an xbox 360 wired controller to play and I really believe the game was designed to be used with a controller as it is also made for the xbox and playstation.

Have him check the sensitivity of his mouse.  Number of pixels per inch. 800 ppi is around the norm. Higher the number,  the more sensitive the swing.

Check main menu -> settings -> controls and make sure DEFAULT GAMEPLAY CONTROLLER and DEFAULT DESIGNER CONTROLLER are both set to KB/MOUSE.
Might also be the LEFT STICK SWING that needs to be set to OFF.

What graphics setting is he playing on? If on high, lower to medium, if no joy, try 32 bit (beta).

ok, He holds down mouse button and the cursor disappears(as it should)
He pulls back on mouse and the club swings back
as soon as he starts to move the mouse forward, the cursor reappears as though he released the mouse button(but he does not release button)
